A flashback reveals a main character, Alessa, was sexually molested by a school janitor as a child; this is discussed and shown non-graphically but is a core plot point. A female character's clothes and skin are ripped from her body by a monster, briefly showing her bare breasts and buttocks in a violent, non-sexualized context. A group of monstrous nurses wear cleavage-baring uniforms.

Violence and gore in Silent Hill

The violence is pervasive, frequent, and extremely graphic. One character is graphically skinned alive by a monster. Another character is burned to death at the stake. In the climax, a supernatural entity uses magical barbed wire to slice, dismember, and bisect numerous people in a church, with explicit gore. Other scenes include a man being torn in two, a child being burned over hot coals in a ritual, monstrous creatures attacking people, and a woman being beaten by a mob.

Language in Silent Hill

Some strong language is used. The dialogue includes at least one use of "fuck" and several uses of "shit," "bitch," "asshole," and "hell." The profanity is infrequent but present.

Drugs, alcohol, and smoking in Silent Hill

A character is seen drinking alcohol in one early scene. This is the only notable instance of substance use.

Mature themes in Silent Hill

The film explores severe and disturbing themes inappropriate for younger viewers. Central themes include religious fanaticism leading to torture and murder, child abuse and neglect, and revenge. A key plot point involves a young girl being ritualistically immolated by a cult after being ostracized and sexually abused, with her immense suffering creating the monstrous reality of the town.
